Bivariate analyses were analyzed using Fisher's exact tests. Variables with a p < 0.20 by bivariate analysis were included in multivariable model selection. Model selection was conducted using stepwise logistic regression and consideration of 2-way interaction terms. The level of significance was set at α = 0.05
