Decision flowchart for selecting patients who need high-dose VCM. This decision flowchart is constructed to be used for patients with eCCr of ≥50 mL/min. Patients with age of < 58 years and eCCr of 81.3–133.3 mL/min are at higher risk of overdosing than patients with age of < 58 years and eCCr of > 133.3 mL/min when received 3 g/day of VCM. Thus, for these patients, 2 g/day of dose may be considered depending on the patient’s condition
